自閉症診斷及篩檢工具(ADOS及STAT)在台灣地區之效用研究-以2歲以下兒童為例

Background and Purpose: The prevalence of ASD has increased significantly in these few years but it is less than other countries in Taiwan, it might because there is few screening and diagnostic tools of ASD in Taiwan and we have few knowledge about ASD early development and stability. This study is to research the predictive value of Autism Diagnostic Observation Schedule (ADOS) and Screening Tool for Autism in Two-year-olds (STAT) for children under 24 months in Taiwan.
Methods: 40 toddlers with ASD and 40 toddlers with developmental delays (DD) were recruited in this study. ADOS and STAT were administered before aged two and after one year and half, diagnosis was made by a multidisciplinary team.
Results: In STAT, toddlers with ASD performed poorer significantly than toddlers with DD in play, requesting, directing attention and total score and 2.5 is found to be a better cut-off score, it yields a sensitivity of .88, a specificity of .73, positive predictive value (PPV) of .76 and negative predictive value (NPV) of .85. In ADOS, toddlers with ASD score higher significantly in each domain and total score than toddlers with DD. Sensitivity of ADOS is .93 and the specificity is .80 while PPV is .82 and NPV is .94. Motor imitation in STAT and item A1, A3, A4, C2, D2, E1, E2 and E3 in ADOS cannot differentiate toddlers with ASD from DD, which mean that there is some culture differences within these items.
Conclusion: Yet, many of these items were not included in either ADOS or ADOS-2 algorithms. Future studies are needed in case to understand the pattern of ASD and the validity of these two tools in Taiwan.
